Correction of Symmastia Removal of implants and Re-augmentation with Silicone Gel Implants

This patient wanted to remove her current implants and replace them with new implants as well as increase the distance between her breasts (correct the mild symmastia).

This patient traveled to have Dr. Pousti perform her surgery. She searched the internet and found that he was very experienced with revisionary breast surgery and symmastia repair. She wanted to fix the mal-position of her breast implants.

Markings were performed before surgery.

 


 

 

Dr. Pousti does his best to plan his surgery and improve the patient’s implant mal-position as much as possible. He will perform an internal suturing technique to improve the position of the implants without any additional scarring on the breasts.


The patient wears a supportive bra and tape after surgery to support the areas that were repaired.

 

The patient is only a few weeks out from surgery with a beautiful result and very happy with the size and shape of her breasts. Notice that the nipple-areola complex are no longer pointing outward but rather straight ahead.

Removal of Breast Implants

This 26 year old patient from Lake Elsinore, California had breast implants placed (by another surgeon) 1 month before coming in to meet with Dr. Pousti. She was VERY UNHAPPY with her breast augmentation result and wanted the breast implants removed. Dr. Pousti spent a lot of time with this patient but after extensive conversation, they decided to proceed with removing her breast implants and not replacing them at all.

This patient was very unhappy with the shape and symmetry of her new breasts and just wanted them removed.

Dr. Pousti does his best to explain the potential risks and complications to his patients.

To make a long story short, I came to Dr. Pousti to have my breast implants removed. After seeing the numerous reconstructive cases he had done, I knew he was the doctor for me.

Revisionary breast surgery: XL breast implants

This 71 year old patient from West Virginia found Dr. Pousti on the internet when she was searching for the best Plastic Surgeon to do her revisionary breast surgery . This patient was interested in removing her current breast implants and replacing them with much larger breast implants.

After meeting with the patient multiple times and getting medical clearance from her primary doctor, Dr. Pousti decided to proceed with surgery. Dr. Pousti placed 800cc saline breast implants and overfilled them to achieve the results this patient wanted.

This is a 41 year old patient was interested in breast augmentation surgery. She wanted to go “to a DD cup” so Dr. Pousti placed 800cc silicone gel breast implants under the muscle for this patient. A year out of surgery, she found that it was it was difficult for her to continue running and exercising so she decided to “downsize” her breast implants. She had her 800cc implants removed and replaced with 500cc silicone gel breast implants. She is currently a “full C- small D” and very happy with her results.

Dr. Pousti met with the patient several times to discuss the procedures that would need to be performed to achieve her results. She and Dr. Pousti decided to proceed with her surgery.



Symmastia Repair with High Profile Silicone Gel Breast Implants

This 22 year old patient traveled from outside of California (Mid-west) to have Dr. Pousti perform her symmastia repair. She had previosly had breast augmentation surgery by another surgeon with a very poor result. She was very unhappy about the medial displacement of her breast implants (symmastia) which caused her nipple-areola complexes to point outward.

She did her research online to find the best plastic surgeon for symmastia repair and that is how she found Dr. Pousti in La Jolla, California. She contacted the office via e-mail and Dr. Pousti called her to discuss her options with her. She felt very comfortable with his expertise on the subject. She also was comforted by the many before and after photos on his website.

This patient met with Dr. Pousti the night before surgery and that is when the markings were performed. As Dr. Pousti performed the markings, he discussed the procedure in detail with the patient. She currently had 550 cc silicone gel breast implants and wanted her current implants replaced with larger 600cc high profile silicone gel breast implants. She had performed her homework and knew exactly what she was looking for.

In the operating room, Dr. Pousti removes the breast implants and corrects the pockets to prevent any further medial displacement of the new breast implants. Internal suturing technique is used to correct the pockets without any additional visible scarring on the patient’s breasts.

She will meet with Dr. Pousti a few times prior to flying back home. She is very pleased with her results so far.

Revisionary Breast Surgery: Implant Mal-Position

This 25 year old patient from New York found Dr. Pousti searching Google for “best revisionary breast surgeon “. After reviewing his website, she decided that for her 4th breast surgery , she was going to have the BEST Plastic Surgeon take care of her and so she flew in to La Jolla, California to meet with Dr. Pousti. She was impressed that Dr. Pousti is double board certified as well as his experience with symmastia repair. She was very unhappy with the medial displacement of her breast implants therefore making her nipple-areola complexes point outward and also the bottoming out of the breast implants . Dr. Pousti discussed her options with her and also discussed the internal suturing technique that would be used to correct the pocket position of the implants and once the implant placement is corrected, the nipple-areola complexes would also be centered on the breasts.

After thorough discuss regarding what would be performed, this patient and her mother decided that Dr. Pousti was going to be her Plastic Surgeon and that they wanted to fly out to California for surgery. This patient met with Dr. Pousti 2 days prior to surgery to discuss the details of surgery, especially the internal suturing technique. She felt very confident with Dr. Pousti’s experience.

Markings were performed – the red marks on the breast indicate where the pockets need to be corrected internally. Dr. Pousti takes great care to achieve symmetry for this patient.

This patient is only a few days out from surgery but already sees the remarkable difference. She is asked to keep the tape and the supportive bra on for as long as possible to help with the stabilization of the corrected area.

She is very happy with the symmastia correction . Dr. Pousti used the patient’s current implants (he did not exchange them).

Here she is a month out of surgery. She is still healing and the implants will continue to settle. Notice the separation of the 2 breasts as well as the correction of the “bottoming out” of the breast implants.

The day of my surgery was almost like a spa day! I went to the hospital early that morning, where they put me in a robe, under a hot blanket, in a reclining chair while I waited. For such a big day, they kept me extremely calm and relaxed. Finally, Dr. Pousti came in and spoke to me again. He re-did all his drawings and told me everything was going to come out great. After surgery, I woke up in a recovery room. I have to say I had very little discomfort or nausea (which is a common problem of mine). For the next few days Dr. Pousti and his staff kept in close contact with me at the hotel I was staying at. They were always there to answer any of my questions, and to check up on me several times a day.
After staying in California for a week after surgery, I saw Dr. Pousti for my final visit. I had to wear some bandages and a V-bra, and he said everything was healing wonderfully. I went home and followed his strict instructions…no stretching my arms too far, keeping the tape dry, wearing the V-bra, etc. Throughout the next 8 weeks I sent Dr. Pousti some pictures of myself as I was healing. He, or someone from his staff, always got back to me right away, and they were all very happy with my results. After 8 weeks, I took off all the bandages, tape, and V-bra. I sent Dr. Pousti my photos with a few questions. He got back to me and sat on the phone with me for over a half hour going through each and every question I had. My results are truly amazing! I honestly never thought I’d be able to correct the symmastia, or get rid of the crinkles. I am proud to say that both are now completely corrected thanks to Dr. Pousti! He gave me hope when no one else even wanted to try to correct my situation. The best thing I ever did was take that trip to San Diego. He truly is one of a kind, from his skill to the way he is with his patients. Never in all the visits I’ve made to different plastic surgeons, did I ever meet a doctor who made me feel so confident and understood. I only wish I’d found him sooner!

This 23-year old patient from Seattle, Washington was interested in extra-large breast augmentation surgery. She was unhappy with the size and shape of her breast implants which were done by a previous plastic surgeon.

She found Dr. Pousti on the internet when she searched for “Best Plastic Surgeon in Southern California”. She wanted a surgeon who had performed larger breast implants and liked the results she saw on Dr. Pousti’s website.

She met with Dr. Pousti and discussed her goals. In order to achieve the look that she wanted, Dr. Pousti used 800cc saline breast implants made by Allergan Corporation filled to 1290cc on the left breast implant and 1340 on the right breast implant.

After continuous communication via e-mail and phone, the patient decided to fly in to San Diego and meet Dr. Pousti.

After 10 years of having her first breast augmentation surgery, the patient decided to go through with the breast implant revision / exchange surgery. Revision Breast Surgery: XL Breast Augmentation

This 23-year old patient from San Diego, California was interested in revisionary breast surgery. The patient was unhappy with her breast size from her previous breast augmentation / breast lifting surgery in 2008 (not performed by Dr. Pousti). The patient did some research for another Plastic Surgeon to revise her surgery and found Board Certified Plastic Surgeon, Dr. Pousti. The patient explained that she wanted to be as big, round, and full as possible.She wanted XL breasts.

Dr. Pousti achieved her goal look by using 800cc, overfilled to 850cc for less chance of rippling, saline-filled breast implants on both breasts made by Allergan Corporation.

The patient is now 3-months post-op surgery and is very happy with her results.

I had my first breast augmentation/lift in May 2008. I had 600cc silicone high profile under the muscle implants. I got to size 32DD but I felt I wasn’t big enough. Those first implants were too evenly proportionate with my height and weight, and instantly I knew I wanted to go bigger. Plus my breasts weren’t as round as I wanted; they were more “cylinder shaped” with little inner and outer cleavage.

Almost a year later I had my second breast augmentation and an areola reduction, this time with dr. pousti. I got 800cc saline moderate profile under the muscle implants…..but the catch was that these implants were overfilled to 1130cc’s. I went almost twice as big with my second surgery. I am now a 32DDDD, or in short, a 32G. I asked for more inner and outer cleavage, which I got. I am much rounder and fuller than I was after my first augmentation. Recovery was much shorter the second time around. After about a week I was able to shower and dress myself with no assistance (it was about 2 weeks with my first surgery). I only took prescription percoset the day I had my surgery; the following days I only took ibuprofen. My back really hurt the first month due to the extra weight in my chest, so I would take half a percoset for that.

I originally wanted to stick with silicone in case there was a future leak, but I was told that oversized implants are only available in saline. I was nervous at first about the overfilling of the implants with saline and all the complications that might happen and having it potentially pop and leak and then ill have one boob and one leaky, wilty boob….but dr. pousti and his staff reassured me that would not happen.

Massaging was difficult at first. I am not sure if it is because they were overfilled or because they were filled with saline, which is harder than silicone, or for both reasons. The first couple weeks were tough and they were hard. I thought they’d never soften. But after a while they did, and now they are as soft as my silicones were. I still massage them every day to keep the pocket loose. If I don’t, they look and feel “constricted”.

I do see myself going bigger in the future. Dr. Pousti went as big as he could. My one dissatisfaction is that my areolas are still a little bigger than what I wanted. I hope in a few years ill be able to go larger again with 800cc’s, or that the market will make larger, custom implants.

Revision Breast Surgery: Correction of Symmastia

This 36-year old patient from Yuma, Arizona was interested in revisionary breast surgery. The patient previously had her breast augmentation performed in 2000, but due to capsular contraction and symmastia, the patient looked into revisionary breast surgery. She researched for a Board Certified Plastic Surgeon and found Dr. Pousti on the Internet.

The procedure of revision breast surgery (internal suturing to help with pocket correction and symmastia repair) were discussed with the patient.


In order to achieve her desired look, Dr. Pousti removed her old implants; corrected the medial mal-position (symmastia) and used 350cc silicone-gel breast implants on bilaterally, made by Mentor Corporation.
