Revisionary Breast Surgery: Correction of “bottoming out” and “lateral displacement of implants” with Silicone Gel Breast implants

This 29 year old patient from Arizona found Dr. Pousti on the internet and came in for a consultation regarding revisionary breast surgery. She had a prior breast augmentation surgery but the results were not what she had expected. She had “bottoming out” of the implants and “lateral displacement” of the implants. In order to improve the size, shape, and the placement of her breasts, Dr. pousti recommended using silicone-gel implants to give her the fuller and larger look she wanted to achieve. 

This patient came in for her pre-operative appointment and markings the day prior to surgery.

Dr. Pousti takes his time and marks the areas with different colors to show what needs to be corrected.

You can see that before her revisionary surgery, her implants did not sit in the correct position on her chest.

Here is a picture taken in the operating room showing one side being corrected while the other side has not yet been corrected. You can already see the difference between the sides.

She had 752cc silicone gel implants made by Allergan Corporation placed. She is only 10 days post-op surgery but very pleased with her results.

Revisionary Breast Surgery: Correction of Symmastia

This 38 year old patient from Guanajuanto, Mexico found Dr. Pousti on the internet and knew that he was the Dr. to help her achieve her goals. She had multiple breast operations and currently had significant implant mal-position (significant asymmetry, symmastia and highly unsatisfactory cosmetic results). Symmastia – commonly referred to as “breadloafing” or “uni-boob”, occurs when two implants touch one another in the center of the chest. If the horizontal muscle that is connected to the sternum and goes across the implant is cut during surgery, then the implant can move toward the middle of the chest. Symmastia results from overly aggressive attempts to alter chestwall anatomy trying to increase cleavage in patients. This outcome is made worse by use of larger implants in thin patients, and is a problem for implants over or under the muscle, though submuscular implant placement allows the muscle to provide some softening of the transition to the cleavage area from the augmented breast mound.

She and her husband took the time to travel to Dr. Pousti’s La Jolla Plastic Surgery office for their consultation.

After meeting with Dr. Pousti, they both felt very comfortable with their decision to proceed with revisionary breast sugery.

Markings were performed the night prior to surgery and the procedure was discussed in detail with the patient and her husband. The “red” area denotes the separation that will be corrected.

In the operating room, Dr. Pousti took care to achieve the best result for the patient.

Tape and a special bra were used to stabilize the cleavage area and support the internal sutures.

This patient wanted to achieve the largest size possible without compromising the symmastia repair. 600cc high profile Silicone Gel implants were used on this patient to achieve her results. She is only a month out from surgery and extremely happy with her results.

This nurse from Temecula, California (Riverside County) came in to see Dr. Pousti after she had breast surgery by another surgeon but with very poor results. She was impressed with Dr. Pousti’s training and experience. She wanted her breasts revised with better size and symmetry.

She and Dr. Pousti discussed that there would have to be a breast lift performed on the right side to achieve as much symmetry as possible.

Markings were performed the morning of surgery and discussed with the patient and her husband.

Revision of Breast Augmentation with Silicone-gel breast implants

This 28 year old patient from Denver, Colorado was interested in enhancing having revisionary breast surgery. She was not happy with her current silicone gel implants. She didn’t have enough fullness and didn’t like the shape of the breasts. She found Dr. Pousti’s website on the internet and flew out to meet with Dr. Pousti from Denver. She flew out 2 times prior to surgery and she and Dr. Pousti discussed her options in detail. After meeting with Dr. Pousti, she decided to proceed with bilateral revisionary breast augmentation with silicone gel implants.

Immediately after surgery, you can see a significant difference in the shape, size and fullness of the breasts.

This patient flew back to Denver, Colorado one week after surgery so these photos are very fresh but we will continue to follow up with the patient via e-mail.

I have a petite frame overall, but my upper body and chest particularly was way smaller than my lower body which is on the curvier side. My goal for breast augmentation to begin with was to have my upper body match my hips, thighs, and behind. As far as surgery, I was very pleased with the staff at Alvarado hospital. All made me very comfortable and eased my nervousness. To say the least, my surgery and experience was a blissful breeze. My results are so wonderful just after two weeks I can only imagine in the weeks to come how gorgeous they will be. My new breasts are moderate profile 700cc which give me the fullness up top which balances out my whole body. GOAL ACHEIVED! New bra size is now 30G. Revision Breast Surgery

This 39 year old patient from Spring, Texas found Dr. Pousti online and started her consultation process via e-mail. She was unhappy with her current saline implants and wanted to have a larger, fuller look as well as better placement of the implants. She saw that Dr. Pousti took care of many patients in need of revisionary breast surgery and felt very comfortable with Dr. Pousti’s expertise.

She had mal-position of her breast implants. She wanted them higher on her chest wall and she wanted the shape and size to be improved. She was also interested in exchanging her saline breast implants for silicone gel breast implants. Another concern of hers was that her right areola was significantly larger than the left so she wanted to get the right areola reduced.

This 32-year old patient from Chicago, Illinois was interested in breast revision / breast reconstruction surgery. The patient previously had breast augmentation surgery in 2003 in Ft. Lauderdale, Florida and revisionary breast surgery in 2004 in Beverly Hills, California but was still unhappy with the shape and placement of the breast implants on her chest.

The patient was unhappy with the cosmetic results of her surgery and after waiting several years, the patient decided to try to find the best plastic surgeon for revisionary breast surgery have breast revision surgery once more.  The patient researched for the most experienced Board Certified Plastic and Reconstructive surgeon and found Dr. Pousti on the Internet.  After numerous contacts via e-mail and phone conversations with Dr. Pousti, the patient decided to fly into San Diego, California to meet Dr. Pousti in person. The patient was able to speak and meet with current patients of Dr. Pousti, who had revisionary surgery so that the patient could feel comfortable with her choice of surgeons.   The patient decided to proceed with surgery, the pre-op was done the same day and the patient flew back in to San Diego  3-weeks later to have her breast augmentation revision performed.

In order to achieve her desired look, previous breast implants were removed, capsulorrhaphy of both breasts and internal suturing of the pockets was performed and  500cc silicone-gel moderate plus profile breast implants were used.

Revisionary Breast Augmentation/Breast Lifting Revisionary/Abdominoplasty Surgery

This patient had previously had multiple procedures done by a different surgeon. She was very unhappy with the results of her  abdominoplasty (tummy tuck) surgery as well as breast augmentation and breast lifting (mastopexy) surgeries. She felt that her scars were too thick and dark. Her breasts were uneven (asymmetry). She wanted larger, fuller breasts…. After researching for a board certified plastic surgeon online she found Dr. Pousti. This 37-year old patient from Valliant, Oklahoma was interested in implant revision surgery. The patient was status post breast augmentation surgery in March of 2009 performed by another surgeon.  The patient claimed her previous surgeon was very conservative and did not want to use implants any larger than 500cc.

Breast Augmentation Revision

This patient from Oregon was looking into revisionary breast surgery. She was not happy with the shape of her breasts and how her breasts looked in a bathing suit.  Her current breast implants were placed “sub-glandular” (above the muscle).  When she met with Dr. Pousti, they discussed placing the implants “sub-muscular” (under the muscle) and to use silicone gel breast implants to get the best natural result.

I don’t think words can ever convey the gratitude I feel towards Dr. Pousti. But I will try. Two years ago I had breast augmentation by a PS whose work I had actually seen on a couple of women. I had wanted to do this for years; unfortunately, I did not do any research and went in with blind trust. Within three months, the left implant slipped out of the pocket. Any time I would slightly recline or lay fully on my back, the implant slid into my underarm. I lived with it for a month more and finally went back to the original surgeon. He said he could fix it so 7 months after my original BA I went in. I knew the minute I woke in recovery something was wrong. It hurt worse than the original BA. When he took the bandage off the next day I wanted to cry. He cut through (I’m pretty sure, even though I’m no doctor) all my tissue on my chest and pushed the implant into the midline of my chest. My left nipple was now on the side of my breast. He admitted to “over-correcting” and said if it didn’t get better he would try to fix it. I should have walked away, but I didn’t. So, 4 months later, he tried to correct the position. It was unsuccessful. And this time because of the damage he did to the tissue down the center of my chest, I had a lot of rippling. I hated to even look in the mirror, let alone have my boyfriend see it. Then to make matters worse, the doctor said there was nothing he could do about the placement of my nipple and that I should just remove the implants all together because no one would ever make me happy. That’s when I found Dr. Pousti. Google search after Google search turned up Dr. Pousti time and again. I made the appointment and knew right away he was the one that could fix it. I had three appointments at his office before the surgery. I was always made to feel so comfortable by all the ladies in the office. (Daisy, you are a dolly, so nice always) Dr. Pousti would humbly reassure me time after time that he would make it better. I was so nervous the days leading up to my surgery. But that day, everyone at Alvarado was so nice and made me feel so comfortable and confident it would work out. One of the things that really stick out in my mind on my surgery day is that when he walked into the room to meet with me and make his marks, he introduced himself to my boyfriend as Tom Pousti. Never once did he even use the word Doctor. He is such a humble man. I admire him tremendously for that. There was never any sign of ego.
