Only after 2 months of her surgery, the patient noticed that she had developed symmastia and was very unhappy with her results and especially the &#8220;uniboob&#8221; look (the implants were touching down the mid-line).  Notice that the patient no longer has &#8220;cleavage&#8221;. The patient&#8217;s nipple-areola looked displaced to each side. The implants have also &#8220;bottomed out&#8221;.</p>

She wanted to make sure that this time she went to a board certified Plastic Surgeon who scecialized in symmastia repair and revisionary surgery.</p>
<p style="padding-right: 5px; padding-left: 5px; padding-bottom: 0px; text-indent: 20px; padding-top: 0px;">Another friend recommeded Double Board Certified Plastic Surgeon, Dr. Tom Pousti in San Diego, so she met with him for a consultation. She was very impressed by the amount of time that Dr. Pousti spent with her explaining how the revisionary surgery would proceed. Dr. Pousti explained that he uses an &#8220;internal suture&#8221; technique therefore leaving no additional scars on the outside of the breasts. She was shown pictures of other patients who also had symmastia repair performed by Dr. Pousti.</p>
<p style="padding-right: 5px; padding-left: 5px; padding-bottom: 0px; text-indent: 20px; padding-top: 0px;">This patient proceeded with revisionary surgery 2 months after her first consultation with San Diego plastic surgeon, Dr. Tom Pousti. Her surgery took about 2 hours with a lot of time and care taken to make sure that we achieved her goals. After her current implants were removed, Dr. Pousti placed internal sutures to reattach the skin to the chest wall and then placed her new implants under the pectoralis muscle. She has smooth, round, saline filled implants placed under the muscle. She has a 390cc saline implant placed on the right and a 420cc saline implant placed on the left.</p>

This patient proceeded with revisionary surgery with San Diego plastic surgeon, Dr. Tom Pousti.</p>

The encapsulation on the left was worse than the right side. The areola were laterally displaced on both sides. The patient experienced numbness of the left nipple-areola complex.</p>

<p>The patient was unhappy with the cosmetic results of her surgery and after waiting several years, the patient decided to try to find the best plastic surgeon for revisionary breast surgery have breast revision surgery once more.  The patient researched for the most experienced Board Certified Plastic and Reconstructive surgeon and found Dr. Pousti on the Internet.  After numerous contacts via e-mail and phone conversations with Dr. Pousti, the patient decided to fly into San Diego, California to meet Dr. Pousti in person. The patient was able to speak and meet with current patients of Dr. Pousti, who had revisionary surgery so that the patient could feel comfortable with her choice of surgeons.   The patient decided to proceed with surgery, the pre-op was done the same day and the patient flew back in to San Diego  3-weeks later to have her breast augmentation revision performed.</p>
<p>In order to achieve her desired look, previous breast implants were removed, capsulorrhaphy of both breasts and internal suturing of the pockets was performed and  500cc<a href="http://www.poustiplasticsurgery.com/Procedures/Procedure_breastImplant.htm" target="_blank"> silicone-gel moderate plus profile breast implants</a> were used.</p>

Very soon after her first surgery, she underwent a second operation on the right side for to remove scar tissue from aroudn the implant (capsulectomy).</p>

She also wanted to address the problem of the right implant being too close to the surface. She understood that her implants had &#8220;bottomed out&#8221; (her nipple-areola complexes were pointing up) but she did not want to address this issue at this time.</p>

He also performed right breast reconstruction using a 6 by 16cm piece of <a href="http://www.revision-breastsurgery.com/revisionary-surgery/alloderm.htm">Alloderm Regenerative Tissue Matrix</a> to protect the right implant and prevent it from coming too close to the surface.</p>

<p>I&#8217;m writing this for anyone considering breast augmentation surgery, or anyone who is coming to Dr. Pousti for revision surgery. I moved to San Diego in August of &#8216;06 and new that finding a doctor to do my revision surgery was a main priority for me. I had lived with a previous complication for two years with increasing difficulty as it kept getting worse and worse. The hardest part of the process for me was learning to trust a surgeon again after my past experience. I&#8217;m 23 and had already had two surgeries, and the amount of emotional stress I had for the past two+ years had worn me down. I found Dr. Pousti online and came in for a consultation within a couple weeks of moving here. After meeting with him, looking at pictures online, and reading patients testimonials, I knew he was the man for me!! (Not to mention his wonderful staff)<br />
I had a pretty serious/complicated issue (take a look at the before and after pics!) I was pretty much hopeless that I would ever be anything but deformed&#8230; and yet Dr. Pousti. didn&#8217;t turn me away, instead he worked with me and did research on new techniques in breast revision surgery using <a href="http://www.revision-breastsurgery.com/revisionary-surgery/alloderm.htm">AlloDerm Regenerative Tissue.</a> He went out of his way to help me, and treated me like and individual person instead of a paying customer. One thing I would like to add because I think it is a common misconception with female patients, is about male surgeons. I know that after having a male doctor for my first two surgeries I was frustrated because I felt like men didn&#8217;t take the time to listen to me, and (with my young appearance) treated me like a child. So before meeting Pousti I had consulted with a female surgeon in the area thinking maybe that was the answer&#8230; I was wrong, she was actually worse than my previous surgeon, and made assumptions about what I wanted and didn&#8217;t want without asking me. In short, It&#8217;s not about a male doctor or a female doctor, it&#8217;s about the person.<br />
I would strongly recommend him to anyone considering cosmetic surgery, honestly he is a very talented surgeon. After coming in for multiple consultations, she felt very comfortable proceeding with revisionary breast augmentation surgery. Dr. Pousti informed the patient that silicone gel breast implants have now become FDA approved and in her case, may give her the results that she is striving for. She wanted Dr.Pousti to enhance the size of her breasts and achieve more cleavage and fullness while still maintaining the natural look.</p>
<p style="padding-right: 5px; padding-left: 5px; padding-bottom: 0px; text-indent: 20px; padding-top: 0px;">Silicone gel implants give more &#8220;natural&#8221; feeling breasts especially for those who have minimal breast tissue. Silicone gel implants also produce less &#8220;rippling&#8221; than saline filled implants. The implants are placed sub-muscular (under the pectoralis muscle) but the muscle does not completely cover the implant and so if a patient has little breast tissue, she may feel the implant ripple on the outer or inner edges.</p>
<p style="padding-right: 5px; padding-left: 5px; padding-bottom: 0px; text-indent: 20px; padding-top: 0px;">She is now only one month after revisionary breast augmentation surgery. Her subglandular (over the muscle) saline breast implants were removed and replaced with silicone gel breast implants placed in the sub-muscular (under the muscle) position.</p>

She had <a href="http://www.revision-breastsurgery.com/revisionary-surgery/Procedures/Procedure_symmastia.htm">symmastia </a>(the implants wer touching down the middle of her chest) &#8212; sometimes called &#8220;uni-boob&#8221;. She also had &#8220;bottoming out&#8221; of the implants. <strong>Symmastia</strong> &#8211; commonly referred to as &#8220;breadloafing&#8221; or &#8220;uni-boob&#8221;, occurs when two implants touch one another in the center of the chest. If the horizontal muscle that is connected to the sternum and goes across the implant is cut during surgery, then the implant can move toward the middle of the chest. Symmastia results from overly aggressive attempts to alter chestwall anatomy trying to increase cleavage in patients. This outcome is made worse by use of larger implants in thin patients, and is a problem for implants over or under the muscle, though submuscular implant placement allows the muscle to provide some softening of the transition to the cleavage area from the augmented breast mound.</p>

<p><em>I came in for my consult and was blown away by the difference in treatment I received from the very start. The waiting room has a spa feel to it. The girls that work in the office are all amazing and take wonderful care of us patients. Dr. Pousti sat and talked with me longer than my other doctor saw me the whole time I was his patient. He made me feel comfortable, and although it was hard to trust another doctor, Dr. Pousti gained my trust right away. I was told from the beginning that if he felt he couldn’t fix me that he would tell me. Luckily he said I had a great chance of being fixed. Although it wasn’t 100% there is no one else I would have trusted to do a better job.</em></p>
<p><em>The day of my surgery is still a blur. One thing I do remember is the beautiful flowers I received from Dr. Pousti and his staff. They brightened my day. I don’t remember much of the first week because of the painkillers, but I’m not going to lie- the road to recovery for this procedure is harder than a breast augmentation alone. You’re not supposed to do much of anything for awhile after. Although I wasn’t in a ton of pain, I began to get antsy because it’s hard not being able to lift your arms or pick things up. Although not having to vacuum for an extended period of time has been great, my boyfriend misses having me there to help him clean. </em></p>
<p><em>The difference this surgery has made on my body is amazing. After my first breast augmentation I gained some weight because I was so depressed. I looked even worse because of the way my breasts were placed- almost like a tree trunk. Now, although I still have a little bit of extra weight on me I look curvy- and feel amazing. Thank you so much Dr. Pousti for making me feel like the woman I wanted to feel like over two years ago when I first decided to have this done. She became very concerned about her developing symmastia. <strong><a href="http://www.revision-breastsurgery.com/revisionary-surgery/Procedures/Procedure_symmastia.htm">Symmastia</a></strong><a href="http://www.revision-breastsurgery.com/revisionary-surgery/Procedures/Procedure_symmastia.htm"> </a>- commonly referred to as &#8220;breadloafing&#8221; or &#8220;uni-boob&#8221;, occurs when two implants touch one another in the center of the chest. If the horizontal muscle that is connected to the sternum and goes across the implant is cut during surgery, then the implant can move toward the middle of the chest. Symmastia results from overly aggressive attempts to alter chestwall anatomy trying to increase cleavage in patients. This outcome is made worse by use of larger implants in thin patients, and is a problem for implants over or under the muscle, though submuscular implant placement allows the muscle to provide some softening of the transition to the cleavage area from the augmented breast mound.</p>

			<content:encoded><![CDATA[<p>This 51-year old patient from Carlsbad, California (Northern San Diego County) wanted <a href="http://www.poustiplasticsurgery.com/Procedures/Procedure_breastRevision.htm" target="_blank">breast implant revision </a>surgery.</p>

<p>The patient is status post breast augmentation surgery in 1989, performed by another surgeon. Due to encapsulation (scar tissue), the patient was unhappy with the results of her previous surgery and wanted breast augmentation revision surgery.  After many years of living with the complications, the patient decided to research the best Board Certified Plastic and Reconstructive Surgeon in San Diego and found Dr. Pousti on the Internet. The patient reviewed many of Dr. Pousti’s before and after pictures and decided to meet with Dr. Pousti.  During her consultation, the patient was able to speak with patients who also had revisionary breast surgery with Dr. Pousti. Impressed by Dr. Pousti’s experience, the patient scheduled her surgery.</p>

<p>In order to achieve her desired look, her previous breast implants were removed and re-augmented with 533cc <a href="http://www.poustiplasticsurgery.com/Procedures/Procedure_breastImplant.htm" target="_blank">silicone gel breast implants</a> made by Allergan Corporation.</p>

<p>The patient is now 10-days post-op surgery and is extremely happy she went through with surgery.  She understands that everything will take time to heal and the implants will settle with time as well.</p>

<p>The patient previously had a  subglandular (over the muscle) breast augmentation in 1973 and revision breast augmentation (subglandular) in 1993 — with the same surgeon. Due to several complications, including encapsulation, the patient had been looking into revisionary breast surgery by another surgeon for many years.</p>
<p>Her friend referred her to Dr. Pousti.</p>

<p>In order to achieve her desired look, the previous breast implants were removed and capsulectomy (removal of scar tissue around the implant) was performed.</p>

<p>Dr. Pousti decided to proceed with sub-muscular placement of the new 400cc <a href="http://www.poustiplasticsurgery.com/Procedures/Procedure_breastImplant.htm" target="_blank">silicone-gel breast implants</a> to reduce the possibility of capsular contracture again.</p>
