Correction of Implant Malposition:

This 32 year old patient from San Diego,California researched for a board certified plastic surgeon because shewas interested in correcting her implant malposition. She previously has breast augmentation surgery (by another surgeon) but her results were not optimal. Revisionary Breast Augmentation:Removal of breast implants placed above the muscle and replacement with silicone gel implants placed under the muscle as well as mastopexy surgery.

This 25 year old patient from Oceanside, California had her first breast augmentation surgery October of 2005 by a different plastic surgeon who had placed the breast implants subglandularly (above the muscle). Over time, she noticed that her implants did not feel or look natural. They were hardening and starting to become painful. Unfortunately, she had developed severe capsular contracture, and decided to search for a board certified plastic surgeon in Southern California who would be able to perform her breast augmentation correction and achieve the results that she was looking for and improve her look.

She met with Dr. Pousti multiple times and decided to proceed with revisionary breast surgery. To Anyone Considering Revisionary Breast Surgery:

I had breast augmentation preformed about two years ago by a surgeon in North Carolina. Within a year of the surgery I began to have complications. I noticed a small bump forming beneath the skin on my right breast. Within a year the bump had grown and caused the breast to become misshapen and uncomfortable. I am 29 years old and my first surgery was performed in Mexico City 10 years ago. At first, I loved my breasts. I started with a 32A and ended up with a 34C, but I noticed with time that my breasts started touching each other. I had a “uni-boob”. Symmastia Repair with Silicone Gel Implants

This 20 year old patient was unsatisfied with her results from a previous breast augmentation surgery performed by a different plastic surgeon. She discovered that her surgery resulted in symmastia. Symmastia - commonly referred to as “breadloafing” or “uni-boob”, occurs when two implants touch one another in the center of the chest. If the horizontal muscle that is connected to the sternum and goes across the implant is cut during surgery, then the implant can move toward the middle of the chest. Symmastia results from overly aggressive attempts to alter chestwall anatomy trying to increase cleavage in patients.

The patient wanted cleavage but with her breasts pointing in the right direction. She wanted to come to an expert in symmastia repair so she found Dr. Pousti online and believed that he would e the best surgeon to improve her results.

She came into the office the night before surgery for “markings” and to go over the specifics of her surgery. Dr. Pousti explained to the patient that having to fix the work of another surgeon is very dificult to do but he has a lot of experience in revisionary surgery.

Dr. Pousti takes 30-45 minutes to perform the “markings”. These markings assist him when he is in the operating room to achieve the best results possible.

For correction of symmastia, the procedure can take from 2-3 hours depending on how much work is involved. • An incision is made (usually along the breast crease) to expose the underlying tissue, muscle, and implant.
• The tissue that encapsulates the implant is cut and rolled back to fit into the pocket.
• Tissue is sutured together to hold implant in place.
• The muscle is then sutured internally back in place, connecting the sternum.
• The incision is then sutured close.

Revisionary Breast Surgery: Correction of “bottoming out” and “lateral displacement of implants” with Silicone Gel Breast implants

This 29 year old patient from Arizona found Dr. Pousti on the internet and came in for a consultation regarding revisionary breast surgery. She had a prior breast augmentation surgery but the results were not what she had expected. She had “bottoming out” of the implants and “lateral displacement” of the implants. In order to improve the size, shape, and the placement of her breasts, Dr. pousti recommended using silicone-gel implants to give her the fuller and larger look she wanted to achieve.

You can see that before her revisionary surgery, her implants did not sit in the correct position on her chest.

Revisionary Breast Surgery: Correction of Symmastia

This 38 year old patient from Guanajuanto, Mexico found Dr. Pousti on the internet and knew that he was the Dr. to help her achieve her goals. She had multiple breast operations and currently had significant implant mal-position (significant asymmetry, symmastia and highly unsatisfactory cosmetic results). Symmastia – commonly referred to as “breadloafing” or “uni-boob”, occurs when two implants touch one another in the center of the chest. If the horizontal muscle that is connected to the sternum and goes across the implant is cut during surgery, then the implant can move toward the middle of the chest. Symmastia results from overly aggressive attempts to alter chestwall anatomy trying to increase cleavage in patients. This outcome is made worse by use of larger implants in thin patients, and is a problem for implants over or under the muscle, though submuscular implant placement allows the muscle to provide some softening of the transition to the cleavage area from the augmented breast mound.

She and her husband took the time to travel to Dr. Pousti’s La Jolla Plastic Surgery office for their consultation.

After meeting with Dr. Pousti, they both felt very comfortable with their decision to proceed with revisionary breast sugery.

Markings were performed the night prior to surgery and the procedure was discussed in detail with the patient and her husband. The “red” area denotes the separation that will be corrected.

Tape and a special bra were used to stabilize the cleavage area and support the internal sutures.

This patient wanted to achieve the largest size possible without compromising the symmastia repair. 600cc high profile Silicone Gel implants were used on this patient to achieve her results. She is only a month out from surgery and extremely happy with her results.

 

Revisionary Breast Surgery

This nurse from Temecula, California (Riverside County) came in to see Dr. Pousti after she had breast surgery by another surgeon but with very poor results. She was impressed with Dr. Pousti’s training and experience. She wanted her breasts revised with better size and symmetry.
