Symmastia Repair with Silicone Gel Implants

This 20 year old patient was unsatisfied with her results from a previous breast augmentation surgery performed by a different plastic surgeon. She discovered that her surgery resulted in symmastia. Symmastia - commonly referred to as “breadloafing” or “uni-boob”, occurs when two implants touch one another in the center of the chest. If the horizontal muscle that is connected to the sternum and goes across the implant is cut during surgery, then the implant can move toward the middle of the chest. Symmastia results from overly aggressive attempts to alter chestwall anatomy trying to increase cleavage in patients.

The patient wanted cleavage but with her breasts pointing in the right direction. She wanted to come to an expert in symmastia repair so she found Dr. Pousti online and believed that he would e the best surgeon to improve her results.

She came into the office the night before surgery for “markings” and to go over the specifics of her surgery. Dr. Pousti explained to the patient that having to fix the work of another surgeon is very dificult to do but he has a lot of experience in revisionary surgery.

Dr. Pousti takes 30-45 minutes to perform the “markings”. These markings assist him when he is in the operating room to achieve the best results possible.

For correction of symmastia, the procedure can take from 2-3 hours depending on how much work is involved. Board Certified Plastic Surgeon, Dr. Pousti takes his time in the operating room to make sure that he does what he can to achieve the patient’s results. The repair of symmastia requires additional surgery to move the implant back into the pocket of original placement.
• An incision is made (usually along the breast crease) to expose the underlying tissue, muscle, and implant.
• The tissue that encapsulates the implant is cut and rolled back to fit into the pocket.
• Tissue is sutured together to hold implant in place.
• The muscle is then sutured internally back in place, connecting the sternum.
• The incision is then sutured close.
